The source is from the root tuber of Euphorbia kansui T.N. Liou ex T. P. Wang, family Euphorbiaceae. It is collected either when the stem and leaves wither in the end of autumn or before bloom next spring. It is skinned and dried for use.

[ Medicinal Properties ]: Bitter, cold, toxic; relate to lung, kidney and large intestine meridians.

[ Medicinal Efficacies ]: Purge water and expel fluid; resolve the swelling and disperse the mass.

[ Usage and Dosage ]: 0.5-1.0g. It can be added to pill or powder form for oral use. The appropriate amount of the raw is used externally . It must be prepared with vinegar for oral use, as this reduces its toxicity.

[ Precautions ]: It should be contraindicated in pregnant women or weak patients. It is incompatible with Gancao.

[ Side Effects ]: Its toxicity is intense. Continuous seven-day intravenous administration can induce poisoning changes of tissues in the heart, liver and kidney. Injection can induce hemolysis. Overdose can induce abdominal pain, drastic diarrhea, water-like stool with tenesmus, nausea, vomiting, dizziness, headache, palpitation, blood pressure descending, deprivation of body fluids, dyspnea, delirium, cyanosis. Even it can induce respiratiory and circulatory failure to death.
 
[ Clinical Applications ]: 1. Edema, tympanites, fluid retention in the chest and hypochondrium       It can be powdered singly for oral use or used togethr with Daji, Yuanhua and Dazao, such as in Shizao Decoction.
 
2. Epilepsy of wind-phlegm syndrome    It can be pounded into powder that is added to a pork heart to be stewed, and then it is prepared with Zhusha into pills for oral use, such as in Suixin Vermilioin Pill.
 
3. Pyocutaneous disease and carbuncle      The powder is mixed with water for external application. For mastadenoma, it is used together with Qinghetaozhi, Sanqi and Shenggancao for external application in modern clinic.

甘遂为大戟科植物甘遂Euphorbia kansui T.N.Liou ex T.P.Wang的干燥块根。春季开花前或秋末茎叶枯萎后采挖,除去外皮,晒干。生用或醋制用。
      
【药性】苦,寒。有毒。归肺、肾、大肠经。       
 
【功效】泻水逐饮,消肿散结。       
 
【应用】        1.水肿,臌胀,胸胁停饮。本品苦寒性降,善行经隧之水湿,泻下逐饮力峻,药后可连续泻下,使潴留水饮排泄体外。凡水肿、大腹臌胀、胸胁停饮,正气未衰者,均可用之。可单用研末服,或与牵牛子同用,如二气汤(《圣济总录》);或与大戟、芫花为末,枣汤送服,如十枣汤(《伤寒论》)。另可与大黄、阿胶配伍治疗妇人少腹满如敦状,小便微难而不渴,如大黄甘遂汤(《金匮要略》)。       
 
2.风痰癫痫。本品尚有逐痰涎作用。临床上以甘遂为末,入猪心煨后,与朱砂末为丸服,可用于风痰癫痫之证,如遂心丹(《济生方》)。       
 
 3.疮痈肿毒。本品外用能消肿散结,治疮痈肿毒,可用甘遂末水调外敷。现代临床用化瘀膏(青核桃枝、参三七、甘遂、生甘草)外贴,治疗乳腺肿瘤。       
 
【用法用量】入丸、散服,每次0.5~1g。外用适量,生用。内服醋制用,以减低毒性。       
 
【使用注意】虚弱者及孕妇忌用。不宜与甘草同用。       
 
【古籍摘要】        1.《神农本草经》:“主大腹疝瘕,腹满,面目浮肿,留饮宿食,破癥积聚,利水谷道。”        2.《本草衍义》:“专于行水,攻决为用。”        3.《珍珠囊》:“味苦气寒,苦性泄,寒胜热,直达水热所结之处,乃泄水之圣药。水结胸中,非此不能除,故仲景大陷胸汤用之,但有毒,不可轻用。”       
 
【现代研究】        1.化学成分:含四环三萜类化合物α-和γ-大戟醇、甘遂醇、大戟二烯醇;此外,尚含棕榈酸、柠檬酸、鞣质、树脂等。       
 
2.药理作用:甘遂能刺激肠管,增加肠蠕动,造成峻泻。生甘遂作用较强,毒性亦较大,醋制后其泻下作用和毒性均有减轻。甘遂萜酯A、B有镇痛作用。甘遂的乙醇提取物给妊娠豚鼠腹腔或肌内注射,均有引产作用。甘遂的粗制剂对小鼠免疫系统的功能表现为明显的抑制作用。所含甘遂素A、B有抗白血病的作用。       
 
3.临床研究:据报道,以甘遂为主或适当配伍还可用于治疗其它多种疾病,如:用甘遂与蟾蜍、砂仁、木香、鸡内金、焦山楂组成甘蟾砂仁合剂治疗肝硬化腹水68例,总有效率为94.8%(天津中医,1989,2:18);用甘遂3g,大黄、芒硝各9g,水煎服,治疗6例结核性渗出性胸膜炎,其中胸水少量者4例,服6-9剂胸水消失(上海中医药杂志,1983,1:26);用甘遂甘草汤(甘遂、枳壳、赤芍、昆布各10g,甘草5g)煎服,治疗7例小儿睾丸鞘膜积液,均愈。(四川中医,1990,7:26);另外,尚可用治关节肿大型类风湿性关节炎、肠梗阻及多型性红斑。       
 
 4.不良反应:甘遂的毒性作用较强,连续静脉给药7天,可见心、肝、肾的中毒性组织学改变。甘遂注射液有很强的溶血作用。本品内服过量,其中毒反应为腹痛,剧烈腹泻水样便,呈里急后重感;如服量较多,可出现霍乱样米汤状大便,并有恶心、呕吐、头晕、头痛、心悸、血压下降、脱水、呼吸困难、脉搏细弱、体温下降、谵语、发绀等症状;可因呼吸循环衰竭致死。
