 It is the mature fruit of Crataegus pinnatifuda Ege. Var.major.N. E. Br. or Crataegus pinnatidida Bge., family rosaceae. The medicinal material is mainly produced in Shandong province, which is best in quality. It is collected at the end of autumn when the fruits are mature, after which it is cut into pieces and dried. It can be used in its raw or the stir-baked form.

[ Medicinal Properties ]: Sour, sweet and slightly warm; relate to splee , stomach and liver meridains.

[ Medicinal Efficacies ]: Promote digestion and relieve dyspepsia; invigorate blood and disperse blood-stasis.

[ Clinical Applications ]: 1. Indigestion         It is widely used for various syndromes of indigestion as epigastric fullness and distention, especially for indigestion caused by improperly eating meat and fat. It can be decocted alone or combined with Shenqu and Maiya to enforce the efficacy of promoting digestion and relieving dyspepsia

2. Postpartum lochiorrhea and abdominal pain with blood-stasis       It is mild in invigorating blood and dispersing blood stasis without aggravating  bleeding. It can be used alone or combined with blood-activating and stasis-dispelling medicinal. When used for abdominal pain caused by  blood-stasis, it is often combined with Chuanxiong, Taoren and Honghua.

Besides, in modern clinical practice it can also be used for coronary heart disease, hyperlipemia and hypertension.

[ Usage and Dosage ]:  9-12g. The raw is better for promoting digestion, while the charred is better for relieving diarrhea and dysentery.

山楂本品为蔷薇科植物山里红Crataegus pinnatifida Bge.var.major N.E.Br.或山楂C. pinnatifida Bge. 的成熟果实。主产于河南、山东、河北等地,以山东产量大质佳。多为栽培品。秋季果实成熟时采收。切片,干燥。生用或炒用。

【药性】酸、甘,微温。归脾、胃、肝经。        【功效】消食化积,行气散瘀。       
 
【应用】        1.饮食积滞证。本品酸甘,微温不热,功善消食化积,能治各种饮食积滞,尤为消化油腻肉食积滞之要药。凡肉食积滞之脘腹胀满、嗳气吞酸、腹痛便溏者,均可应用。如《简便方》即以单味煎服,治食肉不消。若配莱菔子、神曲等,可加强消食化积之功。若配木香,青皮以行气消滞,治积滞脘腹胀痛,如匀气散(《证治准绳》)。       
 
 2.泻痢腹痛,疝气痛。山楂入肝经,能行气散结止痛,炒用兼能止泻止痢。治泻痢腹痛,可单用焦山楂水煎服,或用山楂炭研未服,如(《医钞类编》)方;亦可配木香、槟榔等同用。治疝气痛,常与橘核,荔枝核等同用。       
 
3.瘀阻胸腹痛,痛经。本品性温兼入肝经血分,能通行气血,有活血祛瘀止痛之功。治瘀滞胸胁痛,常与川芎、桃仁、红花等同用。若治疗产后瘀阻腹痛、恶露不尽或痛经、经闭,朱丹溪经验方即单用本品加糖水煎服;亦可与当归、香附、红花同用,如通瘀煎(《景岳全书》)。       
 
 现代单用本品制剂治疗冠心病,高血压病,高脂血症,细菌性痢疾等,均有较好疗效。       
 
【用法用量】煎服,10~15g,大剂量30g。生山楂、炒山楂多用于消食散瘀,焦山楂、山楂炭多用于止泻痢。       
 
 【使用注意】脾胃虚弱而无积滞者或胃酸分泌过多者均慎用。       
 
【古籍摘要】        1.《新修本草》:“汁服主水利,沐头及洗身上疮痒。”       
 
2.《日用本草》:“化食积,行结气,健胃宽膈,消血痞气块。”      
 
  3.《本草纲目》:“化饮食,消肉积,癥瘕,痰饮痞满吞酸,滞血胀痛。”       
 
【现代研究】        1.化学成分:山楂含黄酮类、三萜皂苷类(熊果酸、齐墩果酸、山楂酸等),皂苷鞣质、游离酸、脂肪酸、维生素C、无机盐、红色素等。      
 
  2.药理作用:所含脂肪酸能促进脂肪消化,并增加胃消化酶的分泌而促进消化,且对胃肠功能有一定调整作用。其提取物能扩张冠状动脉,增加冠脉流量,保护心肌缺血缺氧;并可强心、降血压及抗心律失常;又降血脂,抗动脉粥样硬化,其降低血清胆固醇及甘油三酯,可能是通过提高血清中高密度胆固醇及其亚组分浓度,增加胆固醇的排泄而实现的。另外,山楂还能抗血小板聚集、抗氧化、增强免疫、利尿、镇静、收缩子宫、抑菌等。       
 
3.临床研究:据报道,用山楂或经适当配伍治疗消化不良、小儿厌食症、婴幼儿腹泻、菌痢、腹痛、产后瘀滞等多种疾病,均取得满意疗效。近年临床还报道:服降脂乐(每片含山楂0.5g)治疗中老年高脂血症89例,结果降胆固醇及甘油三酯的有效率分别为69.49%和80.28%,并能改善食欲不振,神疲乏力、血压偏高等症状(河南中医,1990,4∶23);用山楂与葛根提取物制成的心血宁片,治疗冠心病心绞痛186例,有效率89.87%,并有良好的降血压和降血脂功效(中成药,1994,7∶25);每日以生山楂90g煎服(儿童减量),治疗急慢性肾盂肾炎105例,效果良好(陕西新医药,1975,1∶35)。此外,又可治疗肝炎、月经逾期不至、下肢软组织损伤、顽固性呃逆,声带息肉,冻疮等。       
 
4.不良反应:多食山楂可引起胃酸过多,还有因吃山楂过量而造成胃石症和小肠梗阻的报道。市售山楂片对小儿虽有促进食欲助消化作用,但因含糖较多,如食用量大,使血糖维持较高水平,则将影响食欲,久之可造成营养不良,贫血等。故中医认为山楂只消不补,无积滞或脾胃虚弱者应慎用或不用(中国食品报,2001,02.01)。       
 
【其他】研究不同炮制温度,对山楂有机酸和胃肠推进功能的影响,提示用于消食时炮制温度最好控制在160℃~200℃之间为宜(湖北中医学院学报,2001,1∶46)。
